Program provides short-term rehabilitation intervention to adults with primary mental health diagnosis in the community setting.Provides short term rehabilitation intervention to people with a primary mental health diagnosis. The service is client centered and focus is on supporting individuals in developing skills, promoting resilience and personal strengths, and connecting to resources related to productivity, self care and leisure. Services include individual intervention with Occupational Therapy and/or Recreation Therapy with focus on: leisure assessment, planning and education independent living skills wellness planning life skills education and vocational support/planning consultation
