This program offers total care and gradual transition to independent living.

The program is for those who can take part in their rehabilitation and gradual transition to independent living.

Services may include:

  • medication teaching/management
  • job training
  • group therapy
  • crisis management
  • advocating for the client
  • planning for discharge
  • transitional housing
  • ambulatory care

Service Providers May Include
dietitians, doctors, mental health aides, occupational therapists (OTs), psychiatrists, psychologists, recreation therapists/assistants, registered nurses (RNs), registered psychiatric nurses (RPNs), social workers

Adults over age 18 who:

  • have complex mental illness issues
  • need targeted mental health services
  • need more resources, and a lot of support / treatment
  • need rehabilitation and community treatment and support
Referral Needed
