Occupational Therapy Services for Chronic Disease Management

Provides occupational therapy assessment and short term services for adults living with chronic health conditions to maximize participation, engagement and safety in their home and communities. Occupational Therapy assessments are completed over the phone and in the client's home.

Offers assessment and intervention focusing on:

  • Difficulty in the home (fall prevention assessment and intervention, home modifications, provision of home safety equipment, difficulty managing activities of daily living)
  • Difficulty getting in/out of the home (home modification recommendations, mobility assessments including part time power mobility)
  • Chronic disease self-management support (difficulty pacing, managing sleep, maintaining physical activity, managing instrumental activities of daily life)

Eligibility

Service targeted to:

  • adults 18 and over, living at home

Clients who have Home Living or Supportive Living support are not eligible for services.
