Service provides assessment, diagnosis, treatment, and education for people who have a thickened heart muscle (hypertrophic cardiomyopathy).The Hypertrophic Cardiomyopathy Clinic (HCM) is a Nurse Clinician directed clinic with multidisciplinary support dedicated to the management of patients with suspected or confirmed HCM. Plan of care includes interval clinical assessment and follow-up by a cardiologist. Services provided may include: assessment, diagnosis, treatment, and education for people who have hypertrophic cardiomyopathy management of prescription medications ongoing patient teaching lifestyle recommendations family screening teaching coordination of necessary specialist referrals
