Senior's Balance and Conditioning Program

An individualized treatment program designed for seniors who have fallen, are at risk for falling, or are experiencing symptoms of functional decline.

This program is designed for community dwelling seniors who are at risk for falling, who have fallen or who are experiencing symptoms of functional decline. Functional decline is often a mix of physical, psychological, social and functional challenges. This may include difficulty with activities of daily living, mobility, gait and balance.

Eligibility

Eligibility includes:

  • ages 65 years and older
  • referrals for people less than 65 years of age experiencing significant functional decline will be considered
  • sufficient cognitive ability to follow 2-part commands
  • living independently or with family in the community, or in an assisted living
    facility
  • the ability to walk 20 metres with or without a walking aid
  • consistent transportation to attend appointments
