Health Information - Access and Disclosure

Releases individually identifiable health information to patients, authorized representatives, service providers and other third parties in accordance with the Health Information Act.

In accordance with Alberta's Health Information Act, this is a service which enables the release of individually identifiable health information with or without the consent of an individual.

A written request is needed to release personal health information.

Fees

A basic fee of $25.00 is applied to all requests which includes up to 20 pages depending on record format i.e. paper, electronic or microfilm record. Each additional page costs from .25 to .50 cents per page. Additional costs may apply. Fees are applied in accordance with the Health Information Act.

If you are unable to pay the fee, please submit evidence of financial hardship.
