Symptom Control & Palliative Care Consult Program

Program for patients of the Cross Cancer Institute who are having problems with pain or other symptoms from cancer.

A patient sees a registered nurse, pharmacist and physician or nurse practitioner during the consultation appointment. Afterwards, the patient is followed by the team until pain or other symptoms improve. Management is then continued by the patient's primary care physician or nurse practitioner. Team members from other disciplines are involved as needed.

Service Providers May Include
This service provides comprehensive care to adult and youth clients for all non-opioid substance and behavioural addiction concerns to address service gaps and offer quick access alternatives to traditional service models. We utilize a multidisciplinary team approach providing a combination of medical and psychosocial treatment options, including:Safe and effective medication for substance use concernsHome detoxification servicesAccess to addiction support groups and programsShort term outpatient counsellingCollaborative care planning and goal settingReferrals to treatment or withdrawal management (detoxification), community support, long-term and/or other specialty services dietitians, occupational therapists, pharmacists, physicians, physiotherapists, psychologists, registered nurses, respiratory therapists, social workers, speech language pathologists, spiritual care professionals
Eligibility

Patient who is 18 years of age or older, under the care of the Cross Cancer Institute and able to attend a clinic appointment.

Service Access

Referral by a physician or nurse practitioner.
