Cancer Patient Navigation--Advanced Disease Navigators

This program is for patients with advanced disease; the Cancer patient navigators will support patients to access resources needed along their care trajectory including transitioning to community-based and palliative care resources.

The advanced disease cancer patient navigator is available to facilitate access to information, support, and cancer services, for cancer patients with advanced cancer and their families. This support can span from diagnosis through treatment and follow-up, to palliation and transition to community based palliative services including palliative home care and hospice.

Eligibility

Patient who is 18 years of age or older, has advanced cancer under the care of the CrossCancer Alberta.

Referral Needed

Referral must be made by a physician or nurse practitioner or nurse.

An estimated wait time will be provided at the time the referral is received.
