Stroke Prevention Clinic

The clinic focuses on prevention or recurrence of a stroke, and identification and management of risk factors, in clients who have had a stroke or transient ischemic attack (TIA), or those at risk for stroke or TIA.

Services include:

  • individual and group education sessions related to stroke, lifestyle, and risk factors**
  • telehealth appointment sessions- consultations with stroke neurologist, patient education
  • referral to other healthcare professionals or programs as required

** Sessions available at Fort McMurray and Grande Prairie only.

Eligibility

Eligible clients include:

  • those who have had stroke or transient ischemic attack (TIA)
  • those who are high risk for stroke or TIA
Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Fax referrals to appropriate number for service required – Telehealth Consult with Stroke Neurologist, or Patient Education only (only available at Grande Prairie and Fort McMurray).

**Note

Patients may self refer for education only component in Grande Prairie and Fort McMurray

Healthcare provider referral required for Education with Telehealth consult with Stroke Neurologist

Clients can self-refer for individual and group sessions.  Please call the facility to make an appointment.
