Palliative Care - Home Care

Provides in-home care, support, and comfort to people coming to the end of their lives and their families with a focus on managing symptom issues, providing emotional and psychological support.

Works with clients with a progressive, life limiting illness. Provides 24 / 7 support to the patient and the family, services include:

  • assessment and management by Registered Nurse Case Managers for the patients physical symptoms (i.e pain, depression, anxiety)
  • support for patients and family members to cope with emotional, psychological and spiritual concerns with end of life
  • coordinate care for in home support service (i.e. assistance with personal care, caregiver respite, volunteer support)
  • assess and coordinate equipment needs to support patients to remain at home
  • facilitate and coordinate transitions to alternate care settings, i.e. hospice
  • faciliate discussions around end of life decision making (i.e. goals of care)

Hours of Operation
24 hour service

Service is available 24/7 for patients who have been admitted to the Palliative Home Care. Patients waiting for admission must call their health provider or go to emergency.

Service Providers May Include
family physicians, licensed practical nurses (LPNs), occupational therapists (OTs), palliative care physicians, palliative care clinical nurse specialists, physiotherapists (PTs), registered nurses (RNs), respiratory therapists, social workers

Adults with a diagnosed palliative, end of life, illness or diagnosis who have been informed of their palliative condition.

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Patients can access the service through a variety of means:

  • patients or families may self refer by calling Community Care Access
  • an existing Home Care client can contact the assigned Home Care Case Manager

Wait Times

An estimated wait time will be provided at the time of appointment booking.


There are fees for any equipment, supplies or medications that the patient or family my require.

Patients or families may be eligible for AADL (equipment and supplies) or palliative Blue Cross. A small percentage of patients qualify for the Integrated Home Care waiver program for equipment and supplies.
