A comprehensive palliative care service, including a community-based hospice, for children with serious illnesses and their families. The pediatric palliative care service provides an extra layer of support to the child and family, along with their primary teams, throughout the child's illness trajectory.Supports include:
• an interdisciplinary team offering support around new diagnoses of serious illnesses, difficult decision-making, anticipatory grief support, and collaboration in comprehensive care planning
•consultative support for care of the dying child in hospital, hospice or community
• admissions to Rotary Flames House, a hospice for respite, symptom management and end of life care in a home-like setting
• outpatient programming for children and their siblings
• grief and bereavement support for parents and siblings
