Stroke Early Supported Discharge Program

The Stroke Early Supported Discharge Program (SESD) provides client-focused, virtual or in-home therapy for adults with stroke who are referred from hospital upon discharge.

This program can be done at home or through virtual sessions. People in this program get up to 6 weeks of therapy. It is for those with mild to moderate problems who need therapy at home, or for those who cannot go to a rehabilitation center. People in the program help decide their therapy goals. Specialists help them overcome obstacles and get back into their community. If needed, they can also help transition to outpatient rehabilitation.

occupational therapists (OTs), physical therapists (PTs), social workers, speech-language pathologists (SLPs), therapy assistants
Eligibility

This program is targeted to:

  • recent stroke with lasting mild to moderate deficits
  • appropriate rehab goals
  • medically stable with reasons that prevent them from attending an outpatient ambulatory program

NOTE: The SESD Program now completes virtual appointments.
