Acute at Home - Child and Adolescent Mental Health

Voluntary short term mental health support provided to children, adolescents and their families following an emergency room visit.

Staff meet with clients in their homes to provide:

  • emotional or mental health support
  • brief therapy, intervention and problem solving
  • health and wellness education and information
  • referral to other services as needed

Office hours are listed above. Each Clinician has their own schedule as this is an Outreach Team.

Eligibility

Children and Adolescents ages 5 to 17 years of age.

Service Access

Patients are referred to Acute at Home by:

  1. Emergency Departments or Urgent Care Centres (following a mental health assessment)
  2. Mental Health day treatment programs upon discharge
  3. Mental Health inpatient units at discharge

Wait Times

Once a child or adolescent has been referred to Acute at Home, someone from the team will contact the family within 72 hours.
