Emergency Department Transition Clinic

Provides follow up for patients seen in the emergency department that require ongoing care. Care may include wound care, medication management or transitional care until patient can be seen by their family physician. Nurse Practitioner's run daily clinics.

Services offered include wound care and medication management and teaching for any condition needing short term follow up to ensure an effective treatment plan is implemented and evaluated.

Care is offered as a transition until patient can be seen by their family physician. If no family physician is identified the team will assist patient in connecting with a family physician.

A team of Nurse Practitioners and Registered Nurses also complete consultations in the emergency department for frail or complex geriatric patients who may need additional supports to return home- or to most appropriate care.
