Day Treatment Service - Community Geriatric Mental Health

A 12 + week program for geriatric clients with mental health concerns, with diagnosis of depression and/or anxiety. Services available:1. Mental Health Assessment and treatment planning/recommendations
2. Psychiatry follow up
3. Group Programming
4. Individual Case management and psychotherapy follow up
5. Outreach Services
6. Occupational Therapy Services

12+ week program that:

  • is offered to senior clients with mental health concerns. Focus is primarily depression and anxiety
  • includes psychiatric assessment, individual therapy and group based therapy
  • has a large component of group activities helping clients become connected in the community and helping them spend more time outside the home

All groups are informed by the Transtheoretical Model for Change (TTM). Each group is designed to meet participants where they are in the change process, providing support, education, and strategies tailored to their current stage and goals. All groups are client centered, flexible offering, with the goal of meeting client specific treatment needs and goals. Groups include: Aging Strong – Education for Mental Wellness, Aging Well-Skills for Mental Health, Cognitive Behavioral Group Therapy, Wisdom and Wellness – A Support Group for Emotional Wellbeing.

Eligibility

Service targeted to:

  • age 65 or over
  • diagnosis of anxiety or depression
  • SLUMs minimum 22 / 30
