To consult on and provide expert care to children and their families with or at risk of developing unhealthy blood clots.Provides children, teens, young adults, and their families, to safely and effectively manage their blood thinner (anticoagulant/warfarin) therapy. Families whose children require anticoagulation therapy are managed on the Home INR (international normalized ratio) monitoring program by the Nurse Practitioner for the program. Home monitoring is proven to provide better warfarin control. on-line information and resources for children and their families requiring blood thinners or those with a difference in their clotting system. self management support for warfarin therapy. on-line coaching and support - secure patient-specific access which allows for individuals who are on warfarin therapy and performing home INR monitoring to enter their INRs, warfarin dose adjustments and questions for review by the KIDCLOT team.
