Stroke and Brain Injury Early Supported Discharge and Stroke Community Rehab Program

In-home therapy offered to mild-moderate stroke and brain injury survivors to assist them in their recovery immediately after their event and hospitalization. Community rehabilitation is reserved only for stroke survivors.

Stroke Community Rehabilitation (CR) provides in-home therapy to mild-moderate stroke survivors who may not have had stroke rehab after hospitalization (stroke under 1 year ago) as well as any stroke survivor 1 year+ at risk of hospitalization/placement or need a top up of rehab and / or education.

In both programs, clients actively participate in setting goals for their therapy, which may continue up to 8 weeks. Stroke specialists also provide support with overcoming barriers, reintegration within the community and transfer of care to outpatient rehabilitation and/or community services for continued support.

Eligibility

Early Supported Discharge (ESD) must have had a recent stroke (within the past 3 months).


NOTE: Medicine Hat location will also accept acquired brain injury survivors for early supported discharge from hospital similar to the ESD program for stroke noted above.


Community Rehabilitation (CR) must have had a stroke 1 year+ and be at risk for requiring increased levels of care in the community.

Wait Times

An estimated wait time will be provided at the time of appointment booking. Generally ESD program referrals are seen within 1-3 days post discharge from hospital. CR referrals may have a brief wait period (1-2 weeks) depending upon current ESD caseload.
