Stroke Early Supported Discharge

 Location: St. Mary's Hospital

In-home and virtual rehabilitation for stroke survivors.

Stroke ESD provides personalized in-home and virtual rehabilitation to stroke survivors who have been referred from hospital upon discharge. The program focuses on clients with mild to moderate deficits due to a recent stroke.

Community Rehabilitation (CR) provides in home therapy to stroke survivors who are at risk for needing increased care due to a previous stroke (1 year or more)

Both programs offer services for up to 8 weeks. Clients set their own goals and actively participate in their rehabiltation in the home and within the community. Stroke ESD team will help coordinate outpatient rehabilitation and/or community services (if needed) at the end of the program.

Eligibility

Mild-moderate stroke impairments; within 100 km of Camrose and live within the Central Zone

Self referrals are considered.


Fees

Fees may be applicable for specific treatments or supplies.
