Stroke Early Supported Discharged Program

Provides in-home therapy to stroke survivors who have been referred from the hospital after they have been discharged.

The Stroke Early Supported Discharge (ESD) team helps clients return home from hospital earlier following a mild to moderate stroke. The (ESD) team provides a short-term, client-driven, multidisciplinary rehabilitation program in the client's home and community environment, helping to ease the transition from hospital to home to community.

Eligibility

Diagnosis of recent stroke who require intensive rehabilitation, but are medically stable and safe to be in their own home independently, or with limited support.

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Individuals must reside within 50km from Red Deer City limits - and be willing to participate in a minimum of one hour of therapy per day.
