This service helps youth and their families prepare for the transition from pediatric to adult healthcare.This service offers the following transition to adult healthcare supports for patients and their families: Well on Your Way website – Provides information, tools and resources for youth, families and healthcare providers regarding the transition to adult healthcare Teens Talk Transition – Workshops to help youth and their families prepare for the transition from pediatric to adult healthcare. These workshops run for three consecutive Saturdays and are offered twice per year. For information about upcoming workshops inquire by email.

Youth ages 12 years and older who have an ongoing health condition and will be transitioning from pediatric to adult care.
