The Complex Care Support Team serves North Zone Continuing Care clients and their clinical teams, when clients have complex behaviours that influence their care.The Complex Care Support team supports patients who have been referred to the Continuing Care stream (Home Care, Designated Living or Long Term Care). Clients may include persons with Dementia, early onset Dementia, brain injury, developmental disabilities or other conditions that affect cognition or behaviors. Our services include: comprehensive assessment and recommendations care planning support link to other specialty resources staff education and coaching support for transition between care settings

Clients in the Continuing Care stream where behavioral needs cannot be met with a usual Continuing Care level of service.
Each case / referral is assessed on a case by case basis.
