Stroke Outpatient Rehabilitation Team

Provides coordinated outpatient stroke rehabilitation services via a specialized interdisciplinary team that includes Physiotherapy, Occupational Therapy, and Speech Language Pathology.

Clients should expect their progress in the program to be reviewed at regular intervals to ensure they are meeting program expectations

Must demonstrate rehab readiness, as indicated by:

  • physically and cognitively able to participate in an active rehabilitation program
  • motivated to achieve rehabilitation goals
  • has access to appropriate transportation to and from appointments
