Chronic Pain Central Access CAT

This central access referral service provides a single point of entry for patients who require assessments and pain management and are referred to by clinicians.

We provide a referral service for clinicians who need to refer adults to a pain management clinic for issues such as:

• Low Back Pain (with radiculopathy)
• Herpetic Neuralgia
• Fibromyalgia
• Arthritis (osteoarthritis, rheumatoid arthritis)
• Neck pain
• Shoulder pain
• Peripheral neuropathy
• Myofascial pain syndrome
• Spondyloarthropathies (i.e. ankylosing spondylitis)

This service schedules patients into the following clinics for Initial assessment:

• Lifemark Pain Clinic
• Kaye Pain Clinic
• Misericordia Hospital Pain Clinic
