CHOICE Restorative

CHOICE South Restorative is a short-term, intensive rehabilitation program that helps clients improve or maintain their health and independence. This program is guided by a "work with you" approach, where clients are expected to be active participants in setting and working toward functional goals.

Clients will come to the CHOICE Centre Road several times a week. Based on their identified goals and care plan, they can receive the following:

  • medical monitoring and treatment
  • rehabilitation or restorative therapy
  • therapeutic recreation, cognitive and social programs
  • personal assistance with daily tasks like going to the washroom (as needed)
  • health and wellness education
  • meals and snacks

Access
Service Providers May Include
health care aides, licensed practical nurses (LPNs), nurse practioners (NPs), occupational therapists (OTs), physiotherapists (PTs), recreational therapists, registered nurses (RNs), social workers
Eligibility

Primarily focused on age 55+; Requiring a coordinated approach to care because of complex medical, social, functional and / or psychological needs;

  • medically stable (i.e. not at immediate risk of hospitalization);
  • able to tolerate attending the program for 4 to 8 hours
  • able to physically attend the program, potentially multiple days/week.
  • willingness, motivation and ability to participate in Program activities, including goal setting and / or working to achieve goals

Wait Times

Waitlist will be determined by available capacity at the program and number and urgency of referrals

Fees

$15/day for optional meals

Fee reduction can be assessed on a case-by-case basis
