Keeping older adults with complex health issues; healthy, at home and out of the hospital.Provides services for older adults who have complex health issues and want to remain living in their own homes. The goal of the program is to keep clients healthy, at home, and out of the hospital system. Services may include: medical monitoring and treatment by physicians and nurses medications dispensed by the pharmacy physical and occupational therapy social work (and Pastoral Care Services at a Good Samaritan location) recreation and social activities personal assistance with daily tasks, as needed health and wellness education

Standard CHOICE criteria plus the following for the Special Care Program:
Referrals can be made through a Home Care Case Manager or by calling Community Care Access
Waitlist will be determined by available capacity at the program and number and urgency of referrals
There is a monthly fee for CHOICE.
Clients also pay the co-payment for medications not covered by their Blue Cross or other insurance up to $100 per month. Over the counter medications are not included in the $100 co-payment.
This program fee may waived for those on full benefit assistance for Alberta Aides for Daily Living (AADL), receiving Senior’s Benefit assistance, and/or under 65 years of age and on AISH.
