Pediatric to Adult Transition

Provides support to Glenrose Rehabilitation Hospital pediatric patients and their caregivers as they transition from pediatric care to the adult health care system.

Service inludes:

  • helping to navigate the pediatric to adult healthcare transition with supports, tools and resources
  • help addressing concerns of clients, families, and members of the healthcare team
  • information and education (e.g., making good decisions, caregiver stress, career or work options, child care options)
  • referrals to community resources and services (e.g., financial help, accommodation, transportation)

  • Disabled and public parking are available on designated surface lots and in the underground parkade.
  • Public transportation available with bus stops along 111 Avenue in front of the Main Entrance of the GRH.
