Pediatric to Adult Transition
Provides support to Glenrose Rehabilitation Hospital pediatric patients and their caregivers as they transition from pediatric care to the adult health care system.Service inludes:
- helping to navigate the pediatric to adult healthcare transition with supports, tools and resources
- help addressing concerns of clients, families, and members of the healthcare team
- information and education (e.g., making good decisions, caregiver stress, career or work options, child care options)
- referrals to community resources and services (e.g., financial help, accommodation, transportation)
