Addiction & Mental Health - Geriatric Psychiatry Day Program

Community based psychiatric day program for individuals with diagnosed mental health concerns that inhibit their access to and participation in generic community supports and services.

The Multidisciplinary Team (MDT) focus on ongoing monitoring and community based treatment of seniors experiencing mental health and addictions diagnoses. Establishment of ongoing client supports, and education in program and community settings to assist in developing and maintaining individual independence and wellness.

Access
Service Providers May Include
community support workers, psychiatrists, recreation therapists, recreation therapy assistants, registered nurses (RNs)
Eligibility
  • physically and mentally stable to attend 5 hours/ day once weekly
  • cognitively aware and able to participate in therapy groups and therapeutic milieu
  • primary diagnosis of mental disorder other than dementia
  • with or without mobility aid, able to mobilize independently the equivelent of one or more city blocks
  • independent in managing personal needs
  • living independently in the community
