Providing consultative services and working closely with health care professionals, families or persons living with palliative or end of life conditions (Life-limiting disease).Life Consult Service (PEOLC) is comprised of a multidisciplinary team working across all rural and urban settings of care within the South Zone. The team provides palliative care consultative support for adult patients, families, and healthcare teams to help manage complex palliative symptoms related to the patient’s life-limiting disease.
This service is provided at locations of care including acute care and community (home, supportive living, long term care, private assisted living).
Services include:
- an assessment of palliative symptoms (pain, nausea, anxiety, dyspnea, psychosocial, emotional and spiritual concerns) and provides collaborative support in managing complex situations
- education regarding disease progression and the end of life
- recommendations for optimizing quality end-of-life care and support for families faced with the loss of a loved one
- assists with prognostication, support understanding of prognosis, advance care planning and goals of care conversations
- accessing palliative community resources
- supporting patient and family centered palliative approach to care
- information on how to support a patient and family centered palliative approach to care
- facilitates understanding of a palliative approach to care
- explores opportunities for early palliative care
- facilitates transition to alternate care settings such as hospice
- mentors other clinicians
