Heart Function Clinic

Provides outpatient ongoing follow-up care, management, and education for patients living with heart failure.

Services this program provides include:

  • initial and ongoing assessment of heart failure
  • providing individualized patient education to equip self-management of heart failure
  • optimization of heart failure care including medication use and dose adjustments
  • patient / family support
  • ordering appropriate diagnostics / interventions as necessary
  • referrals to specialists and other programs

Eligibility

Referral Criteria:

  • Confirmed diagnosis of Heart Failure
  • Pending or resulted ECHO and NT-ProBNP
Referral Needed

Referrals must meet criteria and be ordered by a physician or nurse practitioner through Connect Care
