Alberta Healthy Living Program - Central Access and Triage

This central access referral service offers individuals with chronic conditions or pain access to health education classes, self-management workshops, nutrition counselling, and supervised exercise, with options for Alberta Health Services (AHS) health care provider or self-referrals.

This is the Central Triage Department for the Alberta Healthy Living Program (AHLP) in the Calgary Zone. Referrals that meet criteria will be processed and patient's will be scheduled into the referred service.

AHLP offers health education classes, self-management workshops, nutrition counselling for adults living in the community or living with a chronic health condition.

Effective April 1st, 2025, nutrition services will focus on digestive health, liver health, and malnutrition. Adults with an eligible nutrition-related health condition, referred by an AHS healthcare provider, can access nutrition counselling with a registered dietitian. Participants may be asked to attend a relevant health education class, if available, before booking a nutrition counselling appointment.

Eligibility

Eligibility criteria vary by service. For this program, the following apply:

  • All patients must have at least one chronic condition to qualify.
  • Adults living with an eligible nutrition-related health condition must have a referral from an AHS healthcare provider to receive one-on-one counselling with a registered dietitian.

This service is internet based and telephone information, all services are offered via Zoom or by phone. Registration is required and by appointment only.

AHLP also offers in-person programming for supervised exercise classes at limited locations.

Healthcare provider referrals as well as patient self-referrals for exercise classes are accepted. A referral from an AHS healthcare provider is required for one-on-one nutritional counselling.
