Alberta Healthy Living Program - Central Access and Triage

This central access referral service offers individuals with chronic conditions access to health education classes, self-management workshops, nutrition counselling, and supervised exercise.

This is the Central Triage Department for the Alberta Healthy Living Program (AHLP) in the Calgary Zone. Referrals that meet criteria will be processed and patient's will be scheduled into the referred service. AHLP offers health education classes, self-management workshops, nutrition counselling, and supervised exercise for adults living with a chronic health condition. Nutrition services will focus on digestive health, liver health, and malnutrition.

Health care provider referrals and self-referrals accepted.

Eligibility

Eligibility criteria vary by service:

Supervised Exercise:

Nutrition Counselling:

  • Patients must be 18 years of age or older and live within the Calgary Zone
  • A diagnosis of a digestive health condition, liver condition, or malnutrition is required

Health Education and Better Choices, Better Health:

  • Participants must be 18 years of age or older and live within Alberta
  • Participants do not need to have a chronic condition
