Complex Care Support Team Central Access and Triage

This central access referral service provides a single point of entry for patients who wish to contact the Complex Care Support Team that serves North Zone Continuing Care clients and their clinical teams, when clients have complex behaviours that influence their care.

The Complex Care Support Team (CCST) provides evidence-informed geriatric consultation services for families and clinicians supporting patients experiencing neurocognitive impairment. The CCST can support clients with a variety of concerns including; cognitive changes, behavioral and psychological symptoms of dementia, functional decline and medication optimization. These services are provided by our Community Outreach Team supporting all of the North Zone or our Clinic Team located in Grande Prairie
