The Provincial POWER (Personalized Osteoporosis Care with Early Recognition) Program is Canada's first virtual outpatient Fracture Liaison Service (FLS) that identifies, investigates, and initiates treatment (3i model of care) for patients with new fragility fractures (FF) possibly caused by osteoporosis or poor bone health.Provides virtual care for people with new broken bones and concerns about bone health or osteoporosis with the aim to prevent future fractures. Offers a digital remote service that assesses and investigates patients' bone health, and provides a personalized bone health care treatment plan developed by nurses and physicians. Program elements may include: Bone densitometry scan Blood tests Medication initiation Education surrounding nutrition, exercise, and lifestyle modifications for bone health promotion Referral to clinical or community services

Patients meeting criteria must be referred by a physician or self-referred by contacting the POWER Program team. Appropriate patients will then be enrolled in the program after eligibility screening. Referrals to the program can be completed by contacting program nurses directly by phone.
Note: No coverage is offered by the program for any program elements including but not limited to prescription medication, diagnostic tests, or transportation related costs.
