A short-term mental health service for individuals experiencing homelessness or are newly housed and are not connected to mental health services. SMHT (Street Mental Health Team) can provide mental health assessments and psychosocial interventions with a goal of providing short-term and transitional support.Service description: Outreach mental health service for unhoused and newly housed people who are not connected with other mental health services within the community. SMHT provides psychosocial assessment and support, mental health assessments, specialist and community referrals, and short term mental-health-focused case management. Services are available by appointment through agency referral or self-referral at SORCe. This program has consulting psychiatry and can make referrals to other programs as needed. The team provides a psycho-educational group called the Wellness Exchange.
