Stroke Prevention Outpatient Clinic
The clinic is focused on early identification and prevention of strokes in clients who are at risk for a stroke or have had a stroke before.The Stroke Prevention Clinic provides patients with follow-up care,management and education if at risk of stroke, or after having an ischemic or hemorrhagic stroke, or transient ischemic attack (TIA), in an outpatient setting.
Services this program provides include:
- Investigation, assessment, diagnosis, management of stroke
- Education and support for patients regarding stroke, risk factors, and further prevention
- Optimization of post stroke care, including medication management, lifestyle changes, rehab programs in our community
- Patient / family support, resources available
- Ordering appropriate diagnostics/interventions, referring to specialists or other programs as needed
