This may include immunizations given at: a pharmacy a doctor's office an urgent care centre an emergency department a private travel clinic any place outside of Alberta Accessing immunization records fromMyHealth Records: Anyone age 14 years and older can register for MyHealth Records and access their immunization records. Parents / Guardians of children and youth under the age of 14 years or Guardians of youth over 14 years and adults can access immunization records through obtaining proxy access. Accessing immunization records from a public health centre:
To make a request for anyone under 18, you must be the legal guardian. You may be asked to show proof that you are the guardian. If you are over 18, you must make the request yourself.
