Complex Ambulatory Rehabilitation Program

This multi-disciplinary up to 12-week program supports clients with complex conditions requiring multiple disciplines on an outpatient basis.

Clients with complex conditions with specific functional goals that requires a multi-disciplinary program, can be seen in this time limited intensive out patient treatment program. Clients needs to be able to attend therapy several times per week and independently find transport to attend. The team works with the client to establish feasible functional goals that can be achieved over a maximum of 12 weeks.

Eligibility

Clients with complex rehab needs requiring a multi-displinary approach, where treatment can be completed in 12 weeks.

Referral Needed

Referrals can be entered through Connect Care. 

Wait Times

3-6 months, depending on prioritization

Fees

No fee applicable for treatment, but some of the therapeutic applications used may be charged to the client.
