Location: North Hill Community Health Centre
The Neonatal Transition Team (NTT) is a community-based program providing support for the family following infant discharge from the Neonatal Intensive Care Unit (NICU) and / or Pediatric Units.Services are provided in home, in clinic or virtually. Supports include the following: information about the infant's health, abilities, and special needs regular assessments of the infant's health, growth, and development support for breast, bottle and nasogastric tube feeding guidance to enhance infant nutrition reflux management home oxygen therapy and respiratory assessment blood pressure monitoring as required liaison with the infant's pediatrician/PCP and specialists such as registered dieticians and occupational therapists referrals to other services and community resources such as social workers, lactation consultants and physiotherapists NTT will support family as needed to a maximum of 16 weeks corrected age (~3-4 months post NICU discharge).

Eligibility in this program is possible if:
Other infants with health issues and under 2 months corrected age must be reviewed and accepted by the NTT team lead.
