Antenatal Community Care Program

A community nursing program offering 7 days a week care, with in-home or clinic visits (as appropriate) for pregnant individuals requiring monitoring and fetal health surveillance for medically stable, high-risk pregnancies.

Services include:

  • Nursing assessment of patients in their homes or at the clinic or over the telephone,
  • Fetal monitoring (non-stress tests) as ordered; starting at >/= 26 weeks' gestation,
  • One-on-one perinatal education on high-risk pregnancy conditions and other health concerns,
  • Teaching on self-monitoring and when to seek urgent or emergency care,
  • Individualized nutrition support with the program Dietitian, including general nutrition, healthy pregnancy weight, special diets, supplement review, and food security,
  • Referrals to community agencies and support resources as needed,
  • Ongoing communication with the Most Responsible Healthcare Provider (MRHP)

This service provides in-home care for antenatal patients who might otherwise require hospitalization. Patients remain under the care of their obstetrician, family physician, or midwife (with obstetrical consultation).

Note

Services are provided in patient homes and occasionally at clinic sites.

Regular business hours are 08:30 - 16:30, 7 days a week (including statutory holidays).

Eligibility

Patients must:

  • be stable with reassuring fetal/maternal status and normal NST
  • have a medically high-risk pregnancy condition that meets admission criteria
  • not work outside of their home
  • have access to a working telephone or cell phone
  • reside within Calgary and surrounding area
  • be referred by Most Responsible Healthcare Provider (MRHP)
  • be willing / able to participate in the program

Fees

No fees for clinical services. Possible purchase of a recommended blood pressure monitor may be required for treatment of hypertension concerns.
