Tertiary Neuro Rehabilitation

Doctor to send a physician transfer summary to one of the program's physiatrists. The client will be assessed to determine their fitness for this program or another program.

This service provides:

  • rehabilitation for patients diagnosed with neurologic injury/illness (i.e. Stroke, Brain Injury, Spinal Cord Injury, multiple sclerosis, Guillain-Barre Syndrome, etc.)
  • comprehensive patient-and-family-centred care
  • diganosis-specifc and rehabilitation-specific education to patients and their families
  • consultation for adult patients at all Calgary hospitals (e.g. Foothills Medical Centre, Rockyview General Hospital, Peter Lougheed Centre, and South Health Campus)

This service is available for patient at least 16 years of age. Lives in southern Alberta (Red Deer and south), and has Alberta Health Care in place or in progress.

Eligibility

Inclusion Criteria:

  1. neurologic diagnosis with a functional improvement and demonstrated improvement
  2. anticipated discharge destination has been identified
  3. demonstrates ability, motivation and willingness to participate in therapies, trials of equipment, discharge planning, abide by unit and hospital regulations, at least three 30 minute therapy sessions per day, up in a sitting position for at least 3 hours consecutively
  4. acute medical issues and abnormal lab values have been addressed and are stable
  5. consults and diagnostic tests for diagnosis or treatment of acute conditions have been completed and reported
  6. diagnoses and prognosis have been discussed with patient or designated decision maker
  7. articulated management plan for the patient’s concurrent medical disorders have been established
