Mental Health Transition team that supports individuals with a diagnosis of a primary psychotic disorder or Mood disorder with a psychotic component. CET transitions them from inpatient Psychiatric Units to the community and bridges them until connected to a longer term mental health clinic.CET is a mental health transition team that works with individuals who have traditionally not connected to Mental Health follow-up due to a multitude of barriers. referrals come from Hospital (typically inpatient Psychiatric Units). No community referrals accepted need to have a diagnosis of a Primary psychotic disorder or Mood disorder with a psychotic component CET will meet with them on the Unit prior to discharge and provide a bridge to a longer term mental health clinic CET provides an intensive outreach support that works with individuals on a Community Treatment Order and manages medication access to Psychiatry to manage medication while awaiting connection to Mental Health Clinic

Referrals are done through Access Mental Health from the Inpatient Units. Referrals need to come from Hospital, be medically stable, able to ambulate on own, have housing and a desire to work on treatment goals.
