Acute Care of the Elderly

A program for senior adults (age 65 and older) who have been admitted to the hospital because of an acute illness.

Assesses and provides care to seniors in the hospital. Also offers rehabilitation and community-based programs.

Eligibility

65 and older or functionally geriatric.

Referral Needed

Acute Care of the Elderly (ACE) Screener is sent to the floor for review by the Transition Team Nurse or Nursing staff.
