Stroke and Geriatric Empowerment Unit

A specialized program for adults who would benefit from an intensive rehabilitation program. 6 to 8 weeks of intensive therapy or 4 weeks dedicated to restorative care.

Provides assessment, treatment, and rehabilitation to clients who have had a stroke or require further rehabilitation therapy following surgicall interventions. Clients who expereince a deficit or a sudden onset of in their daily functioning are also treated on an inpatient basis.

Service Providers May Include
dietitians, doctors, health care aides (HCAs), licensed practical nurses (LPNs), occupational therapists (OTs), pharmacists, physiotherapists (PTs), social worker, speech-language pathologists (SLPs)
Eligibility

Client must:

  • be medically stable with a clear diagnosis and established health issues
  • be able to take part in active rehab for 3 hours per day
  • be able to follow one-step commands
  • have sufficient attention, short-term memory, and insight
  • be willing and motivate to take part in the program
Referral Needed

A health professsional must send a referral to the central intake team which will be reviewed. Direct admits are not accepted from RAAPID, a referral must be filled out and submitted.

Wait Times

The rehabilitation team (Central Intake) will review the referral prior to admission. (See eligibility of entrance to the program)
