Home Care - Adult Services

Provides professional health and support services in partnership with clients and families to optimize their ability to continue to live within their community.

Health services may be provided for:

  • care after surgery
  • short-term and long-term care needs
  • palliative and end-of-life care
  • respite services

Each client will be assessed for need, and services will be provided based on the care needs assessed.

Service Providers May Include
health care aides, licensed practical nurses (LPNs), nurse practitioners (NPs), occupational therapists (OTs), pharmacists, physical therapists (PTs), recreation therapists, registered dietitians, registered nurses (RNs), rehabilitation assistants, respiratory therapists (RTs), social workers
Eligibility

Adults 18 years and older that:

  • have been assessed as requiring a health of support service
  • have a valid Alberta Personal Health Care Number or be eligible for one and have completed the application
  • have an environment that is safe for care providers
Referral Needed

Anyone can be referred to Home Care.
