Social Work (Integrated Access Department)

Provides Social Work support.

Assist patients and address family concerns regarding:

• financial assessment and Interventions to assist patient and family in problem-solving and planning for the financial elements
• family and caregiver coping adjustment support
• mediate and assist with conflict resolution and provide advocacy to support patient and family goals
• decision making capacity process, alternate decision makers and related legal documents
• identify and connect patients with appropriate community supports and resources
