The role of the Social Work department is to consult with other healthcare professionals to address a patient's care needs and provide support to help them deal with illness or injury.Services include: Conducting assessments to inform care plans Educating patients and families on health, treatment, and resources Collaborating with healthcare teams to address barriers and psychosocial factors Offering counseling and therapy for coping, mental health, and addiction Referring to community and government services for ongoing care Facilitating transitions between care settings Engaging in research and advocating for improved healthcare access and quality Advocating for patients, families, and communities when quality healthcare is unavailable or inaccessible
