Integrated Home Care - Consult and Treatment

Provides medical support, nursing, and therapy teams through Community Care Access to help people live safely at home.

Available Home Services Nursing Care:

  • wound management, IV therapy, and health check-ups
  • therapy support: physical therapy, occupational therapy, and mobility training
  • personal assistance: bathing, dressing help, and daily living support
