Palliative and End of Life Care

Provides symptom management, comfort and guidance to people experiencing a life limiting or terminal illness.

Health services offered to palliative and end of life patients include:

  • nursing care
  • physiotherapy
  • occupational therapy
  • respiratory therapy
  • nutritional services
  • social services and counselling
  • advance care planning

Support services offered to palliative and end of life patients can include:

  • personal care
  • respite care
  • Meals on Wheels
  • medical equipment provided by Alberta Aids to Daily Living Program

Services vary by community. Please contact the Home Care program or Acute Care facility in your community for more information, or contact Continuing Care Access.

Hours of Operation

The Continuing Care Access Team has a 24/7 voicemail option and operates: 

  • Monday to Friday 8:15 am to 8:00 pm
  • Saturday, Sunday and Statutory Holidays from 8:15 am to 4:30 pm.
