For people generally 65 years and older.
A referral from a doctor or psychiatrist is needed. People can also be referred after an assessment by a Senior’s Outreach Nurse in the Community Mental Health Centre. Referrals need to include a medical work-up, medication list, and copies of mental status or psychiatric assessments.
