Tertiary Neuro Rehabilitation

The Tertiary Neuro Rehabilitation (TNR) unit focuses on the rehabilitative care and treatment of patients diagnosed with a neurological condition who have the potential to significantly benefit from intensive, time limited, complex and interdisciplinary rehabilitation.

The Tertiary Neuro Rehabilitation (TNR) interdisciplinary team works collaboratively with patients and their families to support the rehabilitation journey by:

  • providing education to patients and families or other close supports
  • teaching patients how to manage and / or direct their care
  • facilitating functional recovery and enabling independence with mobility, activities of daily living, communication and / or community integration
  • counselling patients and families through their injury / illness adjustment
  • assisting patients and families plan for discharge and transition into the community

Doctors to send a physician transfer summary to one of the program’s Physiatrists. The client will be assessed to determine their fitness for this program or another program.

Additionally, Physiatrists provide consultation for adult patients at all Calgary area hospitals (e.g. Foothills Medical Centre, Rockyview General Hospital, Peter Lougheed Centre, and South Health Campus).

Eligibility

Inclusion Criteria:

  1. Neurologic diagnosis with a functional improvement and demonstrated improvement
  2. Anticipated discharge destination has been identified
  3. Demonstrates ability, motivation and willingness to participate in therapies, trials of equipment, discharge planning, abide by unit and hospital regulations, attend at least three 30 minute therapy sessions per day, up in a sitting position for at least 3 hours consecutively
  4. Acute medical issues and abnormal lab values have been addressed and are stable
  5. Consults and diagnostic tests for diagnosis or treatment of acute conditions have been completed and reported
  6. Diagnoses and prognosis have been discussed with patient or designated decision maker
  7. Articulated management plan for the patient’s concurrent medical disorders have been established
Referral Needed

The Tertiary Neuro Rehabilitation (TNR) Program does not accept direct referrals. All referrals must be directed through either a Connect Care consult to a Physiatrist or through Connect Care InBasket message to the Stroke Rehab Coordinator (stroke referrals only).

TNR patients' rehabilitation and medical care needs are supported in collaboration by both a physiatrist and a ward physician (rehabilitation primary care). The physiatrist supports the rehabilitation needs and the ward physician is the Most Responsible Health Practitioner (MRHP) who supports the medical needs. Once a patient is deemed eligible and ready for rehabilitation, they will placed on the waitlist and a member of the team communicate with sending units when a bed becomes available.
