The Geriatric Assessment & Rehabilitation Inpatient Program assesses and treats older adults with medical issues, helping them return to living in their community.

This service provides:

  • Therapy to help you get better
  • Help with managing your medicines
  • Care from nurses
  • Help from social workers

audiologists, dietitians, doctors, feeding and swallowing therapists, health care aides (HCAs), licensed practical nurses (LPNs), occupational therapists (OTs), pharmacists, physical therapists (PTs), psychiatrists, psychologists, recreation therapists (RTs), registered nurses (RNs), speech language pathologists (SLPs)
Eligibility

The service is for older adults who have recently undergone major physical, mental and social decline, usually within the past two to three months.

Clients must:

  • Be able and willing to take part in active rehab (for example, be fit enough to do the activities)
  • Be medically stable and have a desire to return to their community.
